What happens if you lose the Voter ID Card or its stolen? Well, you can apply for a duplicate Voter ID Card. There are 2 ways to apply for a duplicate Voter ID Card, they are Offline/Online.

STEP 1: To apply for a duplicate Voter ID Card, you will have to fill the Form EPIC-002. Get this form from the nearest electoral office.
STEP 2: Fill this form with accurate details like name, address, DOB, EPIC Number and reason for application. Try to fill the details (just as they were present), in the original Voter ID Card.
STEP 3: Attach a copy of the FIR, if the Voter ID Card has been lost or stolen. Attach copies of address and identity proof along with the form and submit to the electoral office.
STEP 4: On submission of the form, the authorities give you a reference number. You can use this number to track the application status.
STEP 5: The electoral officer will verify your details and if accurate, your application is passed.
STEP 6: You will be notified when the duplicate Voter ID Card is ready. You go to the electoral office and collect it.
STEP 1: You must visit the State Electoral Officer’s website and download the Form EPIC-002.
STEP 2: Fill up this form with the necessary details and attach supporting documents like FIR, Address and Identity Proof and so on.
STEP 3: Submit application form with attached documents to the nearest electoral office and collect reference number.
STEP 4: On successful verification, your application is passed.
